Athletic physical and forms for TC athletes

All Tri-Center athletes in grades 7-12 are required to have a sports physical before participating in school athletic activities. Students will be required to have the physical and return the physical examination form, concussion fact sheet, and acknowledgment form below to school before participating in any athletic practices or contests.
